Quality Measures 

At Bend Memorial Clinic we know that our patients value the quality of care we provide.  We have made a commitment to continually provide information about our quality measures and why they are important to our patients’ health and well being. In addition to these Patient Centered Medical Home quality measures, BMC is gathering data for 90+ quality measures that encompasses every department in the clinic.  This process is a high priority for BMC and we plan to publish our results and strategies for continuous improvement in 2011.

This is the first step in the building of our disease registry that will also include quality data tracking of:

  • Mammograms
  • Colonoscopies
  • Dexa Scans 
  • Diabetic eye exams 
  • Diabetic foot exams

*** LDL = % of patients whose latest LDL results were less than 100. 

*** HbA1c = % of patients whose latest A1c results were below 8.0. 

*** BP = % of patients whose last 3 blood pressures averaged less than 140 and less than 90.
