Please
call us as far in advance as possible to schedule your
appointments. This will make it easier to choose times that are most
convenient for you. Some same day appointments are available in Family Practice and Internal Medicine. Please talk with the appointment center to see if your provider has same day appointments available. Same day appointments are available by calling after 7am the day you'd like the appointment.

Remember,
to avoid unexpected fees contact your health plan to be sure your
plan has approved the health care you are requesting from or that is
recommended by BMC.
Checklist
of information you will need when making an appointment
Clinics
need information to identify you, to locate your medical record, and
to reserve the appropriate amount of time for your appointment. We
will ask you for the following information:
- Patient's
name / address / phone number(s)
- Patient's
date of birth or Social Security number
- Patient
insurance information
- Patient
employer information
- Patient
emergency contact information
- Reason
for the appointment
- Whether
you want a particular provider or the first available appointment
Canceling
an appointment If you
cannot keep your appointment, please call BMC as soon as
possible. We realize that sometimes emergencies come up and your
plans may change. Canceling your appointment with less than 24
hours notice, or not showing up for your appointment may result in a
cancellation charge. When you call we will help you reschedule your
appointment. Giving us as much notice as possible helps us
better serve you and our other patients.
