Jeff T. Caba, PA-C

A graduate of Duke University School of Medicine Physician Assistant Program, Jeffrey is a certified Physician Assistant in our Urgent Care Center. Prior to joining Bend Memorial Clinic in 2003, Jeff served as an Internal Medicine Physician Assistant, a sleep lab technician, a physical therapy aide and a diagnostic testing technician.

Teresa Cousineau, PA-C

Teresa is a certified Physician Assistant in BMC’s Urgent Care Center. Teresa earned a BS degree with a Physician Assistant Certification from State University of New York. She also has a BS from Plattsburgh State College in Biology. Teresa has served as a family practice physician assistant for 17 years providing her patients with primary care, women’s healthcare and orthopedic care.

J. Randall Jacobs, MD

Dr. Jacobs has been at Bend Memorial Clinic since 1996 and serves as Physician Director of the Urgent Care Center. Certified by the American Board of Family Practice, Dr. Jacobs graduated from the University of Kansas Medical School in 1972 and completed his residency at the University of Washington in 1975.

Laurie D. Ponte, MD

Dr. Ponte specializes in Family Practice and has been at Bend Memorial Clinic since 1990. Certified by the American Board of Family Practice, Dr. Ponte graduated from Michigan State University Medical School in 1987.

Ann M. Reitan, PA-C

Ann is a certified Physician Assistant serving BMC’s Urgent Care patients. Ann completed her Physician Assistant Program from Duke University School of Medicine in North Carolina. She earned a Bachelor of Science degree in Biology from Willamette University in Salem, Oregon. Ann is a member of the Oregon Medical Association, American Academy of Physician Assistants and the Oregon Society of Physician Assistants.

Patrick L. Simning, MD

Dr. Simning has been at Bend Memorial Clinic since 1996. Certified by the American Board of Family Practice, Dr. Simning graduated from the Uniformed Services University of Health Sciences in 1986 and completed his residency at the Naval Hospital at Camp Pendleton in 1991. Dr. Simning is a staff physician at our Urgent Care Center.

Thomas H. Wendel, MD

Dr. Wendel joined the Clinic in 2002. He earned his medical degree at Oregon Health Sciences University in Portland and completed a residency at University of Wyoming Family Practice. He has earned numerous honors and distinctions in his medical and teaching career including the Lewis and Ruth Carpenter Teaching Award for Teaching Excellence and multiple Mentorship Awards at OHSU. Dr. Wendel also earned recognition as the Oregon Primary Care Association Outstanding Physician for 2001. He is board certified in Family Practice and is a member of the American Academy of Family Physicians.

Brent Wesenberg, MD

Dr. Wesenberg graduated from the Medical College of Wisconsin in 1986 and did his Emergency Medicine Residency at Kern Medical Center in Bakersfield, California 1987-90. Dr. Wesenberg graduated with honors and was twice awarded the President’s Undergraduate Fellowship Research Grant.

Prior to his position with the Clinic, Dr. Wesenberg was the Director of North Shore Emergency Department in Auckland, New Zealand for 2 years. He also spent 14 years in Port Townsend, Washington, as the Director and staff physician for the Emergency Department at Jefferson General Hospital. In addition he has worked at St. Clair Hospital in Tacoma, Washington as an emergency staff physician.
