From kids to adults, tonsillitis to top-to-toe physicals, our Family Medicine physicians make your primary care their primary concern. Trained in preventative care as well as skilled management of illnesses of all types, our Family Medicine doctors are your family’s personal physicians, caring for all ages from babies to adults and providing both outpatient care and care in the hospital. All Family Medicine physicians are certified by the American Board of Family practice, which requires an in-depth examination every six years to ensure up-to-date family care skills. Family Physicians provide direct care and make appropriate referrals to specialists when needed.

Kathleen C. Antolak, MD

Dr. Antolak has been at Bend Memorial Clinic since 2001. Certified by the American Board of Family Practice, Dr. Antolak graduated from the University of Minnesota Medical School in 1981 and completed her residency at the University of Minnesota in 1984. Prior to joining Bend Memorial Clinic, she did medical work in Nepal and Tibet while also working with refugees and immigrants who were survivors of political torture. Dr. Antolak also provides travel consultation and immunizations as part of the Clinic’s Travel Medicine Services Department.

Dr. Joseph Bachtold

Joseph Bachtold, DO, MPH, FAAFP

Dr. Bachtold came to BMC from John Day, Oregon, where he was a Family Medicine physician at the Strawberry Wilderness Clinic.   He currently is a Director for Academy of Family Practice and serves on the Steering Committee and Scientific Review Committee of the Oregon Rural Practice Based Research Network through Oregon Health Sciences University.  He is a Clinical Assistant Professor in the Department of Family Medicine at OHSU. He is also a fellow of the American Academy of Family Practice and has received multiple teaching awards from OHSU and the American Academy of Family Practice.  Bachtold received his Doctor of Osteopathic Medicine at the Western University of Health Sciences in California and completed his Residency through Oregon Health Sciences University in Klamath Falls.  He holds a Masters of Public Health from the University of Hawaii in Health/ Epidemiology.

Jeffrey Boggess, MD

Dr. Boggess has been at Bend Memorial Clinic since 1986. Certified by the American Board of Family Practice, Dr. Boggess graduated from the University of Miami (Florida) Medical School in 1975 and completed his residency at Tacoma Family Medicine in 1980.

Amy DeLoughrey

Amy DeLoughrey PA-C

Amy DeLoughrey, PA-C has joined the Family Medicine department of Bend Memorial Clinic. DeLoughrey received a national certification as a physician assistant and holds a Masters Degree in Physician Assistant Studies from the University of Utah. DeLoughrey also holds a Masters of Public Health from University of Denver and a Bachelor of Arts in Kinesiology from University of Colorado. Before working at BMC, DeLoughrey worked at Bend Surgical Associates as a Physician Assistant.

May S. Fan, MD

Dr. Fan has been at Bend Memorial Clinic since 1990. Certified by the American Board of Family Practice, Dr. Fan graduated from the University of California San Francisco Medical School in 1987 and completed her residency at the Maine Medical Center Mercy Hospital in 1990. Dr. Fan practices at the Sisters Clinic. Dr. Fan also provides travel consultation and immunization as a part of the Clinic’s Travel Medicine Services Department..

Stuart G. Garrett, MD

Dr. Garrett has been at Bend Memorial Clinic since 1986. Certified by the American Board of Family Practice, Dr. Garrett graduated from the University of Kentucky Medical School in 1975 and completed his residency at the Oregon Health Sciences University in 1978. Dr. Garrett also provides travel consultation and immunization as a part of the Clinic’s Travel Medicine Services Department..

Derek B. Hamblin, MD

Dr. Hamblin graduated from Oregon Health Sciences University and completed his residency at Malcolm Grow Medical Center, Andrews Air Force Base in 1998. He received his certification by the American Board of Family Practice in 1998.

Alan C. Hilles, MD

Dr. Hilles has been at Bend Memorial Clinic since 1978. Certified by the American Board of Family Practice, Dr. Hilles graduated from Oregon Health Sciences University in 1970 and completed his residency at the University of Utah in 1976. Dr. Hilles also provides travel consultation and immunization as a part of the Clinic’s Travel Medicine Services Department.

Rachel Lemke

Rachel Lemke, PA-C

Rachel Lemke, PA-C joined the Family Medicine department of Bend Memorial Clinic in 2008. Lemke holds a Masters Degree in Physician Assistant Studies from Oregon Health & Science University and a Bachelor of Science degree in Zoology and Psychology from the University of Wisconsin - Madison.  She also has experience in laboratory research, geriatric nursing care and was previously employed as a Disability Determination Specialist for the Department of Health and Family Services in Madison, Wisconsin.

Charlotte Lin, MD

Dr. Lin joined Bend Memorial Clinic’s Family Medicine team in 2009. She received her Bachelor of Science from Georgetown University and Medical Degree from University of Medicine and Dentistry of New Jersey. Dr. Lin completed her residency at Tufts University and also completed an additional training in acupuncture at UCLA.  Prior to coming to BMC she worked on the Navajo Reservation in Arizona for 3 years. “Taking the time to learn about my patients' background and their value set is important to me. I am then able to collaborate with my patients to develop a healthy lifestyle.  Each person is unique, so a holistic approach provides the greatest benefit,” said Lin. In her free time, Dr. Lin enjoys yoga, rock climbing, biking, running, sailing, skiing, scuba diving, learning new foreign languages and cooking.

Dana M. Rhode, DO

Dr. Rhode graduated from the University of Osteopathic Medicine and Health Sciences in 1998. She completed her residency at the University of Arizona in 2002.

Emilie Rogg, MD

Prior to joining BMC, Dr. Rogg worked as an Emergency Physician at Ventura County Medical Center and at Santa Paula Memorial Hospital. She also worked with group practices in Santa Paula and Ventura, CA, specializing in full-spectrum family medicine and internal medicine. Dr. Rogg also spent time in rural India as a volunteer physician, assisting in OB and primary care. Dr. Rogg graduated from Rensselaer Polytechnic Institute of Troy, NY, with a degree in biomedical engineering and earned her medical degree from University of Connecticut School of Medicine in Farmington. She completed her Family Medicine residency in Ventura and is board certified in Family Medicine.

Hans G. Russell, MD

Dr. Russell joined BMC in 2001. Dr. Russell attended St. George’s University School of Medicine in the Caribbean Islands and completed his residency at Ramsey Family and Community Medicine in St. Paul, Minnesota. He is a member of the American Medical Association and the American Academy of Family Physicians.

Eric J. Schneider, MD

Dr. Schneider joined Bend Memorial Clinic in 2000. Board Certified in Family Practice by the American Academy of Family Physicians, Dr. Schneider graduated from State University of New York at Buffalo School of Medicine and Biomedical Sciences and completed his residency in Family Medicine at University of Virginia Health Sciences Center. Prior to joining Bend Memorial Clinic, Dr. Schneider practiced medicine at Northwest Primary Care in Portland, Oregon.

Cindy Shuman, PA

As a Physician Assistant, Cindy is formally trained in providing diagnostic, therapeutic, and preventative health care services under the supervision of a staff physician.

Cindy has a Masters Degree in Physician Assistant Studies from Oregon Health & Science University, with a specialization in family and community medicine. She holds a Bachelor of Sciences degree from the University of Oregon. Cindy also has experience in clinical research and laboratory work.

Edward M. Tarbet, MD

Dr. Tarbet joined Bend Memorial Clinic in 2001. Board Certified by the American Board of Family Practice, Dr. Tarbet attended the University of Nevada-Reno School of Medicine in Reno, Nevada and completed his Family Practice Residency at Oregon Health Sciences University in Portland, Oregon. Previous to graduate school, Dr. Tarbet was an operating room technician for the United States Navy. Prior to joining Bend Memorial Clinic, he was a Family Practice physician at the Portland Clinic, LLP.

 

John D. Teller, MD

Dr. Teller is an experienced Family Medicine physician from the Portland area. He has worked as Medical and Site Director for Legacy Clinics and as a Family Physician at HealthFirst Medical Group and Tualatin Medical Center. Dr. Teller received his medical degree at Oregon health Sciences University and completed his residency at Lutheran Family Practice in Brooklyn, New York. He also completed a short-term teaching fellowship from Michigan State University. Dr. Teller is Board Certified in Family Medicine.

Thomas A. Warlick, MD

Dr. Warlick has been at Bend Memorial Clinic since 1977. Certified by the American Board of Family Practice, Dr. Warlick graduated from the University of Oregon Medical School in 1971 and completed his residency at the San Bernardino County (California) General Hospital in 1974.
