Getting Registered
Registration is an important step to becoming a BMC patient after you have scheduled your first appointment. Registration includes collecting basic patient information, and creating your medical record. Preparing your registration before the day of that appointment will save you time and ensure all required paperwork has been completed.
We recommend that you print out a Patient Registration Form* to complete and bring to your first appointment. Please also print out and read the "Notice of Privacy Policies" in either English or Spanish.

If you cannot complete the form above, you can register the day of your appointment. However, please allow an extra 20 minutes so you won't miss your appointment.
Checklist of information you will need to register:
- Patient's full legal name, including maiden name and nickname(s)
- Patient's street address, and home and work telephone numbers (or address and telephone numbers for the parents or guardians of minor children)
- Patient's Social Security number
- Patient's birth date
- Health insurance information (name of the insurance company, name of the insured person and his/her place of work, mailing address for claims, and group and policy numbers)
- Patient's Photo ID
- Medicaid or Medicare card (if the patient is on federal or state assistance)
- Name, address and telephone number for person who will be responsible for payments (if patient does not have health insurance)
- Special needs (interpreter, wheelchair access, etc.)
- Date and type of injury (This information is necessary when you are seeking treatment because of an accident while you were at work or involving a vehicle.)
Preparing for your appointment
While BMC will make most of the preparations for your appointment, we do need your help and there are a few things you must do.
- Insurance/Medicaid/Medicare card: Please bring your insurance, Medicaid or Medicare card with you to your appointments. We will verify your coverage, so we can process your bill correctly.
- Co-payment or pre-payment: If your health plan has a co-payment or you have a pre-payment requirement, please bring your payment with you. We accept cash, personal checks, VISA debit cards, Discover, MasterCard and VISA.
- Medications list: Please bring a list of your current prescriptions, over-the-counter drugs, vitamins or supplements, as well as a list of known drug allergies.
- Medical records and X-rays: If you are being referred to a BMC specialist from a non-BMC clinic, please arrange for them to send your medical records and appropriate X-rays to BMC. For additional information, call 541-317-4367.
- Notes about your health concerns: Some patients find it helpful to write a few notes about the symptoms they are experiencing and any other health concerns.
- Directions: If you are new or are using a clinic for the first time, check maps and directions to be sure you know where the clinic is located and where to park.
