At Bend Memorial Clinic, our goal is to ensure that each patient’s individual care is met in the right way, at the right time, at the right location and by the right provider. Our Patient-Centered Medical Home (PCMH) exists for that very reason. This team-based model of care, led by a primary care physician, provides continuous and coordinated care to ensure the highest level of healthcare.
Your physician-led PCMH team works together to provide high quality care, backed by medical evidence, increased access to providers, clear communication and care coordination with your other healthcare providers. This team coordinates all of a patient’s health needs, including management of chronic conditions, behavioral health, visits to specialists, hospital admissions, information about preventative screenings, check-ups, tests and care advice during office hours.
The ultimate goal of the Patient Centered Medical Home model is to achieve the triple aim of increasing quality of care, increasing patient satisfaction and controlling escalating healthcare costs.
