Our Patient-Centered Medical Home (PCMH) exists for that very reason. This team-based model of care, led by a primary care physician, provides continuous and coordinated care to ensure the highest level of healthcare.

Your physician-led PCMH team works together to provide high quality care, backed by medical evidence to increase access to providers, clear communication and care coordination with your other healthcare providers. This team coordinates all of a patient’s health needs, including management of chronic conditions, behavioral health, visits to specialists, hospital admissions, information about preventative screenings, check-ups, tests and care advice during office hours.

The ultimate goal of the Patient Centered Medical Home model is to achieve the triple aim of increasing quality of care, increasing patient satisfaction and controlling escalating healthcare costs.

Does PCMH cost more?

No, it does not cost more to be in Summit Health’s PCMH. Primary Care patients in our internal medicine and family medicine departments benefit from Patient Centered Medical Home care as part of being a Summit Health patient.

Do I have to enroll?

No, patients do not need to be selected into the Summit Health PCMH program. All patients automatically have the principles of PCMH as part of the care delivery at Summit Health.

What is different about PCMH vs. normal care?

A PCMH is a better way to take care of patients in a more comprehensive manner. This team-based model of care, led by a primary care physician, provides continuous and coordinated care to ensure the highest level of healthcare is being extended to the right person, at the right time and in the right location. The care is delivered through a medical team that includes Midlevel medical providers such as Physician Assistants, Nurse Care manager, Receptionists, medical assistants and nutritionists.

How do I know if I need preventative screenings?

Delivering the highest quality of care to our patients also means informing patients about recommended preventative screenings based on your age, medical history and risk factors. We are proud to use the latest technology to determine and automatically inform our patients when they are due for screenings, tests and other preventative measures as recommended to maintain optimal health. In some cases your Medical Home team will be the ones to inform you when screenings are needed as well as our automated calling system to remind you when it is time for important screenings and appointments.

Do I automatically get assigned a Nurse Care manager?

The Patient Centered Medical Home team is designed to provide the right care, by the right provider, at the right time. Not all patients require a Nurse Care manager to monitor their health. However, many patients with a complex set of health conditions will be assigned a Nurse Care manager to assist in their care.

Who do I call to get an appointment?

Please call the Summit Health appointment center at 541-382-4900. As part of our PCMH program we do have same day appointments available for patients with acute conditions.

How do I get a same day appointment via the PCMH program?

When you call to make your appointment ask our appointment center about our same day appointment availability. If a same day appointment is not available with your primary care physician or team, Summit Health’s Urgent Care is open seven days a week and walk-ins are welcome. Summit Health Urgent Care physicians input all details of your Urgent Care visit into your patient record in the Summit Health central electronic health records system. When you follow up with your Summit Health primary care physician, that physician can see the details of your visit to Urgent Care providing a comprehensive overview of vital health information including imaging, lab work and details of any treatment provided.
